Monterey Bay Internet Roaming Service Agreement v1.1 (10/11/99) Monterey Bay Internet provides two options for users that travel outside of our Northern California dialup areas, and would like to access the Internet without paying toll charges for a long distance call. Please check our dialup numbers at http://www.mbay.net/docs/ before using a roaming service to avoid unnecessary charges. These services are charged ONLY if you use the service in a given calendar month; there is no cost to sign up. Please place a check next to the option(s) you wish to have enabled for your account. Both options may be selected. [ ] "Toll Free" 888 Access. You may dial this number from anywhere in the Continental United States. Your account will be charged 10 cents per minute that you are connected, with a $1 per month minimum. This configuration does not require installing any additional software on your computer. [ ] iPass Global Roaming Service. iPass has over 4000 POPs around the world, so you can access your Monterey Bay Internet account with a local or small long distance call from just about anywhere. There is fee of as low as 4 cents per minutes that you use the service, depending on where you are calling from. This requires installing an easy-to-use dialer program on your computer. For more information on iPass, please see the web page at http://www.mbay.net/ipass/. To use the Roaming Services, your entire account must be payed for on a monthly basis with a credit card. ( ) I wish to start paying for my entire account with a credit card, and have submitted a secure Credit Card Update form using the web page at http://www.mbay.net/utils/. The undersigned Account Holder is responsible for the security of the Account, and agrees to pay all charges resulting from the Roaming Services selected above, even if the account is used by someone other than the account holder. The Account Holder may request an itemized list of calls that their account is charged for within 90 days of the charge, but agrees to accept the accuracy of the list in advance.
